$50-$60 per hour Good work/life balance:
Only 40 hours per week, rotating weekends.
13 week assignments give you the flexibility to see if the company is a good fit.
Possible temporary to permanent.
Requirements of Social Worker:
MD LMSW or LCSW -C Must have social work experience in a hospital setting.
Oncology and/or hospice and palliative care experience Discharge planning experience Cerner or EPIC experience 13 week assignment, M-F, 8-4:
30pm, Unit:
Oncology or Med/surg
Job Description:
Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans.
Makes referrals to outside agencies and coordinates continuity of care.
Works collaboratively with the multidisciplinary team to optimize patient outcomes.
Contributes to the attainment of organizational goals.
Will be required to provide coverage during absences, vacancies and on call.
